Payment and Cancelation policies: 

Payment Policies:  In the state of Illinois, insurance plans do not accept Naturopathic Doctors. Cash, check and all major credit cards are accepted.  HSA or Health Flex Spending accounts are accepted. We will provide a receipt to submit.

A Form of payment is required to hold the appointment time but will not be processed. Payment is expected at the time of service.

At Living Tree Natural Health, we believe that receiving naturopathic care is an investment in your health.  Prior to becoming a naturopathic doctor, Dr. Crystal invested her own time in resources to recover her health. We understand the investment and time it takes to recover to optimal health. Please contact the office for current first and return visit fees.

Appointment Policy
Our goal is to provide exceptional quality care in a timely manner. In order to do so we have had to implement an appointment/cancellation policy. The policy enables us to better utilize
available appointments for ongoing care.

You can schedule online for your appointments or call Living Tree Natural Health at 847-214-0204 to schedule with Dr. Crystal.

Cancelation of an Appointment
We understand changing schedules.  In order to be respectful of the needs of the Chicago suburb area community please be courteous and call Living Tree Natural Health promptly if you are unable to attend an appointment. This time will be reallocated to someone who is in urgent need of consult.

If it is necessary to cancel your scheduled appointment we require that you call by 24 hours before your scheduled appointment.  Appointments are in high demand, and your early cancellation will give another person the possibility to have access to timely care.

How to Cancel Your Appointment
To cancel appointments please call 847-214-0204. If you do not reach the receptionist you may leave a detailed message on the voicemail. You may also cancel a scheduled appointment 24 hours in advance and reschedule your appointment for a more convenient time.

Late Cancelations
Late cancellations, made less than 24 hours prior to your appointment time, will be considered as a “no show”.

No Show Policy
A “no show” is someone who misses an appointment without canceling it 24 hours in advance. No-shows inconvenience those individuals who need access to care in a timely manner.

A failure to present at the time of a scheduled appointment will be recorded in the documentation as a “no show”.  We reserve the right to charge the for the late cancel or no show appointment. A letter alerting to the fact that there was failed attempt to show up for an appointment and did not cancel the appointment 24 hours in advance will be sent.
